Unnecessary Deaths and Gross Negligence
Engineering workers work under difficult conditions especially in plant shutdowns when there is very limited time to complete many tasks.
At times people are tempted to take short cuts in order to save time and money and so risks are taken. These short cuts can include not testing for hazardous conditions especially when hot works like welding are concerned.
One report mentioned that they were working on a pipeline
near the methanol tank while another report said they were working on the tank so it seems there are different reports on the same incident.
Whatever the type of work, it seems the safety precautions were sadly missing for hot work in a dangerous location. Both the owner and the contractor are liable for not taking the necessary steps to prevent this tragedy.
